Texas Patient Rights: Records, Licensing & Complaints

As a licensed chiropractic provider in Texas, Rothgeb Recovery & Wellness is required under House Bill 4224 (89th Regular Session, effective September 1, 2025) to provide clear, step-by-step instructions for how patients may request their health care records, contact the appropriate licensing authority, and file a consumer complaint. The information below fulfills that requirement under Section 181.105 of the Texas Health and Safety Code.

You have the right to access and obtain copies of your chiropractic records at any time. To make a request:

1. Submit a written, signed, and dated request to our office. Written requests are      preferred, though verbal requests are accepted.
2. Specify the records you need. For example: "All records from January 2024 to present.”
3. Include your full legal name, date of birth, and a phone number or email address so we can verify your identity.
4. Submit your request by one of the following methods:

1. We will process your request in accordance with applicable Texas law and HIPAA regulations. A reasonable, cost-based fee may apply for copying and mailing.

If you believe your health information privacy rights have been violated, you may file a complaint with the Texas Health and Human Services Commission (HHSC):
